The agreement will cover patients who receive treatment at any of Miramont’s seven clinics in the Denver area. The agreement involves a pay-for-value system, with incentives tied to quality outcomes and care coordination. Through the partnership, providers and Humana will use population health management tools like predictive analytics, disease management and wellness programs.
