Affinia Health expands partnership with Wellcentive for population health

Affinia Health Network of Muskegon, Mich., has expanded its partnership with Wellcentive to use its population health management platform in its Trinity Health Accountable Care Organization.  

 

AHN is a subsidiary of Grand Rapids, Mich.-based Mercy Health and a member of the Trinity Health Accountable Care Organization.

Under the agreement, AHN will implement Wellcentive's PHM platform across its clinically integrated network, including its Grand Rapids, Mich., regional providers, to support data aggregation and value-based care strategies.

In a prepared statement, Affinia Health medical director David Van Winkle, MD, said, "This expansion will further our ability to take advantage of new payment methodologies, redesign and improve care and improve financial outcomes under these new payment models."
