Colorado human services department warns 12,000 of error

Officials from the Colorado Department of Human Services are notifying 12,230 residents that their information may have been exposed due to an error in its assistance programs.

State officials discovered that some notice to reapply letters were sent to the wrong individuals. The letters had information that did not match the individual who received the letter. Individuals have been asked to shred the letters or take them to a Department of Human Services office to be properly discarded.

Information in the letters was limited. No Social Security numbers, addresses or other specific information was affected.

"The state is following all best practices to ensure that no harm results from this error and remains committed to protecting Coloradans' personal information. This includes making credit monitoring available to people whose personal information was exposed," state officials said in a statement.
