University of Chicago Medicine, Valence Health partner for population health

The University of Chicago Medicine has tapped Valence Health, a provider of clinical integration, population health and value-based care solutions and services, for the academic medical center's population health software needs.

Specifically, the University of Chicago Medicine will use the Valence's Vision software as the technology backbone and support of its clinically integrated network. Vision is designed to combine data from physician practice management and electronic health record systems, hospital information systems, lab results, prescriptions and other data sources, to create a comprehensive view of clinical patient care.

Officials from the University of Chicago Medicine hope to use the software to identify patient populations based on conditions, gaps in care per clinical performance guidelines, lab results and other factors to focus the care the medical center provides.
