A Roadmap for Population Health Management: 6 Stages in a New Era of Healthcare

The Institute for Health Technology Transformation has released a whitepaper detailing a roadmap for healthcare providers to automate population health management within their organization.

"Population Health Management: a Roadmap for Provider-Based Automation in a New Era of Healthcare" was developed through a collaborative effort of health, payor, association and software vendor executives and aims to outline six major stages within population health management and describe how automation can be used to make population health management more "feasible, scalable and sustainable," according to the report.

The roadmap includes the following stages:

  • Planning for population health management — setting goals, demonstrating leadership, assessing technology and developing an overall strategy for population health management.
  • Data collection, storage and management — investing time up-front to build integrated and reliable data systems that provide accurate reports.
  • Population monitoring and stratification — identifying subgroups of patients by risk, conditions or other criteria in order to create actionable population registries.
  • Patient engagement — developing outreach tools and campaigns to tailor interventions.
  • Team-based interventions — developing high-performance care teams to coach patients who need them most.
  • Measuring outcomes — analyzing data to report impact on health outcomes, health status and cost.
