Patient-centered analytics: The intersection of big data and population health

Certainly there is no lack of collected data in the healthcare environment. Data are produced and collected from nearly every touch-point in the system. Yet, an important question remains: How do we harness and tame that data in order to learn and move forward to improve health and achieve the triple aim (improved outcomes, increased access, and higher efficiency)?

Not only are we collecting more and more data each day, but the way in which providers are expected to use that data is evolving. Healthcare reform mandates an evolution to value-based care delivery and requires the metrics to accomplish that change. At its core, value-based care is patient-centered care — treating the whole patient, across time and across settings by anticipating and addressing the full spectrum of their health needs. This approach is most often referred to as population health, and the provider industry must interact with all of the large data sets that have been collected over the years in order to perform the patient-centered analytics that will inform the (r)evolution.

Applying a patient-centered lens to large case-level data analysis can help shape ideas and develop initiatives to keep people healthy and out of the hospital — interventions that will help achieve the healthcare triple aim.
