Axial Exchange, iBlueButton, VoIDSPAN Win Safe Transitions Innovation Challenge

Axial Exchange, iBlueButton and VoIDSPAN are the three winners of the "Ensuring Safe Transitions from Hospital to Home" innovation challenge, according to a news release by the Office of the National Coordinator for Health IT.

ONC, HHS' Partnership for Patients and Health 2.0 announced the winners of the challenge, which was part of ONC's Investing in Innovation program. The challenge called for new approaches to improve patient safety and care transitions between hospitals and post-hospital care settings, including the home, nursing home and hospice.

Axial Exchange, the first place winner, was awarded $25,000 for its Axial Care Transition Suite, a web-based application that can communicate information to a patient's next care setting. Humetrix received $10,000 for the second-place iBlueButton mobile application, which provides patients and caregivers with immediate access to personal health information. Flexis received $5,000 for the third-place winner VoIDSPAN, which integrates voice, SMS and web technologies into a mobile app for patients and healthcare providers.
