Survey: What tools do patients want for chronic condition management?

Twenty-seven percent of U.S. consumers with a chronic condition would like to use a monitoring device to track their condition, according to a report by research firm Parks Associates.

Here are four tools for chronic condition management, ranked by consumer interest.

1. Rewards and financial incentives from an employer and/or insurer

2. Health monitoring devices that automatically track a condition

3. Health and medical information in an accessible format of a user's choice

4. Digital or human health coach who uses patient information to create personalized recommendations
