'Practical Playbook' Offers Guides to Integrate Public Health, Primary Care

Bethesda, Md.-based de Beaumont Foundation, Durham, N.C.-based Duke Community and Family Medicine and the Centers for Disease Control and Prevention have collaborated to create the Practical Playbook, an online resource to guide and support partnerships between public health organizations and primary care providers to improve population health and drive down costs.

The playbook is a collection of resources, tools and case studies gathered from various sources on more than 35 topics to present a comprehensive overview of successful integration. Such resources include a five-stage integration model, city community health assessments, comprehensive needs assessment and action plans from other organizations.

Organizations are also invited to upload and share their own success stories and contribute to the collective body of knowledge in the playbook.
