Mercy Health to launch PatientPing's care coordination platform

Cincinnati-based Mercy Health is joining forces with PatientPing, a health technology company focused on patient care coordination.

Mercy Health will deploy the PatientPing platform, using it to connect with hospitals and post-acute care facilities outside of the organization as well as access and share data in real time. The platform will allow Mercy Health to enhance care transitions for Medicare Advantage and Comprehensive Primary Care Plus patients as well as patients within various bundled payment models.

Mercy Health comprises 450 health facilities, including 23 hospitals. The health system will initially rollout the PatientPing platform in the Toledo and Cincinnati regions before expanding to other regions in Ohio and Kentucky.

"Through the PatientPing platform, our clinical care teams now receive real-time information and notifications on their patients."
