The healthcare provider will use the CCM tool to manage care of Medicare patients with chronic conditions. The tool helps providers expand care management beyond face-to-face visits.
“The goal of CCM is to provide Medicare patients with a customized care plan and regular patient engagement through a Care Manager in order to achieve a healthier patient population. The expected outcomes of reduced ER visits and unnecessary hospital stays or readmissions; better control of chronic conditions like diabetes and high blood pressure and improved medication monitoring will ultimately help reduce the cost of healthcare,” said Jeffry Michael Duke, executive director of Medical & Surgical Clinic of Irving.
