The hacker accessed the computer system of Onsite Health Diagnostics, the third-party contractor used by the state of Tennessee. Potentially compromised information includes names, birth dates, address, phone number and gender, according to the report.
Onsite has not yet received reports of identity theft, though it is still offering free identity theft protection services for one year.
