Twenty-three U.S. hospitals received an “F” in The Leapfrog Group’s fall 2025 safety grades — including 11 that were downgraded from a “D.”
Leapfrog assigns hospital safety grades twice a year using up to 30 performance measures, primarily from CMS and its own voluntary survey. The ratings are intended to assess preventable harm, but some hospitals have criticized the methodology as flawed and not reflective of an organization’s quality and safety performance.
In April, the Palm Beach Health Network — which operates Delray Medical Center — filed a lawsuit accusing Leapfrog of assigning artificially low scores to hospitals that don’t participate in the survey and using incomplete data in a way that harms reputations. The group filed a motion for summary judgment in October, reiterating claims that the ratings are “dangerous and misleading” and designed to pressure hospitals into participating in its survey.
Leapfrog has denied the allegations, saying its grades are based on transparent, evidence-based methodology using CMS and survey data to assess preventable harm.
