Partners Connected Health, Hitachi to create AI-driven technology that helps reduce readmissions

Partners Connected Health, part of Boston-based Partners HealthCare, joined forces with Hitachi to develop artificial intelligence technology that can predict the risk of 30-day hospital readmissions for heart failure patients.

The technology assists providers selecting patients for readmission prevention programs. It uses deep learning to construct a risk prediction model and can also explain to users why certain patients were identified as high risk.

Partners Connected Health simulated the use of the technology among heart failure patients participating in the Partners Connected Cardiac Care Program. They found that the prediction algorithm achieved a high accuracy, helping to reduce the number of patient readmissions.

Partners Connected Health and Hitachi will together conduct a prospective study for the technology.
