The program will start with hypertension, congestive heart failure and diabetes patients, with COPD to be added later in 2023. The initiative, which spans six states, will remotely monitor patients’ vital signs and provide virtual care as needed.
“At Ardent, we’re looking at everything through the lens of the consumer — putting people at the center of the care model,” Ardent President and CEO Marty Bonick said in a joint Jan. 6 news release. “Healthcare is complex, but doing things the way we’ve always done them doesn’t serve our patients or caregivers.”
Cadence, a General Catalyst portfolio company, has previously partnered with Brentwood, Tenn.-based Lifepoint Health, Franklin, Tenn.-based Community Health Systems, and Louisville, Ky.-based ScionHealth.
