Health IT tip of the day: Optimize patient monitoring based on individual needs

Patients’ needs vary between populations and individuals. So, too, should the methods providers use to deliver care and interact with them.

What works well for one patient, could be unworkable for another. Some patients adapt easily to technology, others do not. Some have deep support networks, others have little to none. Some patients are highly engaged and knowledgeable about their conditions, others are inconsistent or uncertain how to properly apply the knowledge they have been given. The key is in optimizing patient monitoring based on the needs and preferences of various population segments. That’s the only way to effectively activate and engage them toward the desired outcome.
