The incident occurred within Wolfe’s EHR platform myCare Integrity, which was provided by clinical services company Eye Care Leaders. The unauthorized access took place on or around Dec. 4, 2021, when the third party deleted myCare Integrity databases and system configuration files, according to the Sept. 21 Wolfe Eye news release.

The breach was part of a nationwide targeting of eye care practices. The third party may have had access to patient names, addresses, dates of birth, social security numbers, diagnostic information and insurance information.

Wolfe Eye has not received any alerts of identity theft or data misuse. However, they have notified potentially affected patients and are providing a telephone line to answer any questions.
