The hospital was recognized for its efforts to reduce racial disparities among Black patients with heart failure. It created artificial intelligence-based tools to help identify and manage high-risk patients and opened a new clinic to address their social and behavioral health needs. Since 2022, the hospital has reduced the readmission rate gap between Black patients and the general heart failure patient population by 5.4%. Overall, readmission rates for heart failure patients fell from 33% to 20%.
The Joint Commission and Oakland, Calif.-based Kaiser Permanentete created the health equity award in 2021 to honor the system’s late Chairman and CEO Bernard Tyson. The award recognizes healthcare organizations that achieve measurable and sustained reductions in healthcare disparities, according to a Dec. 19 news release.
