Optum ACO (Phoenix). Formed in 2014, Optum ACO is the collaboration between Phoenix-based Banner Health Network and UnitedHealthcare. Banner coordinates and manages care for more than 50,000 UnitedHealthcare policyholders. This year, Optum ACO was selected to participate in CMS’ Next Generation ACO initiative.
