OneCare Vermont (Colchester, Vt.). Dartmouth-Hitchcock in Lebanon, N.H., and the University of Vermont Medical Center in Burlington launched OneCare Vermont in 2013. The ACO is one of the country’s largest by physician network size. OneCare Vermont was chosen as Vermont’s exclusive partner in a Medicaid initiative that aims to mirror CMS’ Next Generation ACO model. OneCare is an MSSP ACO.
