Hackensack Meridian Health expands hospital at home to 5 hospitals

Edison, N.J.-based Hackensack Meridian Health is expanding hospital at home to two more New Jersey hospitals after treating more than 1,000 patients with the care model.

The health system is extending the program to Brick-based Ocean University Medical Center and Stafford Township-based Southern Ocean Medical Center with plans to grow hospital at home to all 18 of its hospitals.

“This is a whole new healthcare experience for patients and their caregivers that will deliver better clinical outcomes and patient satisfaction while expanding access to patients who may avoid treatment for serious conditions because they don’t want to go to the hospital,” said Patrick Young, president of population health at Hackensack Meridian Health, in a July 7 news release.

The program, a partnership with hospital-at-home company Medically Home, already operates out of Hackensack University Medical Center in Hackensack, JFK University Medical Center in Edison, and Jersey Shore University Medical Center in Neptune.

Under the care model, acute care patients receive hospital-level treatment at home, through in-home technology and medical equipment, telehealth appointments with physicians and twice-daily in-person visits from nurses.

Hackensack Meridian’s program accepts Medicare and most major commercial payers.
