VitalCare Home Health Services offers remote cardiac care

VitalCare Home Health Services, a Cheboygan, Mich.-based affiliate of McLaren Northern Michigan in Petoskey, is set to expand its telehealth program, according to Charlevoix Courier.

Thanks to a grant from the Charlevoix County Community Foundation, VitalCare Home Health Services will be able to equip cardiac patients who have been discharged home with telehealth monitoring units. These monitors transmit data to the VitalCare Telehealth Nursing Team for off-site monitoring of vital signs and symptoms for chronic heart failure, chronic obstructive pulmonary disease and hypertension.

Last year, only 3 percent of VitalCare Home Health Services patients enrolled in the telehealth program were readmitted to the hospital following discharge, according to Charlevoix Courier.
