According to CMS, care transition refers to the movement of a patient from one setting of care to another.
For the report, researchers surveyed 86 healthcare organizations.
The top five risk factors that healthcare organizations consider when identifying care transition candidates, according to the report, are as follows. Respondents could choose more than one risk factor.
• Frequent hospitalizations — 82.3 percent
• Complex chronic conditions — 82.3 percent
• Frequent emergency room use — 74.2 percent
• Cognitive impairment — 40.3 percent
• Long-term care patient — 35.5 percent
