N.J. Hospital Association Launches Consumer Website for Heart Failure Patients

The Institute for Quality and Patient Safety, a non-profit affiliate of the New Jersey Hospital Association, has launched a consumer-oriented website designed to help heart failure patients better manage their condition and prevent hospital readmissions.

The website offers up-to-date information, advice, resources and links for heart failure patients, including a search tool to help find local heart failure support groups and walking paths near patients’ homes. Healthcare providers are also encouraged to share this resource with patients.
