The Joint Commission of Pharmacy Practitioners released an updated version of the Pharmacists’ Patient Care Process, replacing the original framework established in 2014.
The update is the result of a two-year review led by a group of representatives from 14 national pharmacy organizations, according to an Aug. 19 news release from JCPP. The group used a consensus approach to assess the 2014 framework, including a decade of practical use and feedback from public comment.
The changes include a stronger focus on pharmacist expertise in medication management, person-centered care and health equity. The update also addresses shifts in healthcare delivery, including expanded scopes of practice and team-based care.
JCPP will establish an implementation task force to support adoption of the new model, the release said.
