The agreement aims to improve care coordination, results and cost for more than 17,000 Aetna Medicare Advantage members in Western Pennsylvania.
Under the agreement, a care management team will use data to initiate care interventions, education and preventive care for Aetna Medicare Advantage members. The goal of the collaboration is to reduce costs while increasing access to medical services and improving patient experience.
The Warrendale, Pa.-based hospital network was formed in 2017 by four nonprofit health systems: Butler (Pa.) Health System, Greensburg, Pa.-based Excela Health System, St. Clair Hospital in Pittsburgh, and Washington (Pa.) Health System. The CIN comprises more than 1,450 hospital beds and over 1,000 employed and affiliated physicians.
Bridges Health Partners inked a similar deal with Humana in February.
