2. Iowa — 133 hospitals, 33 ASCs (4.0)
3. Montana — 64 hospitals, 16 ASCs (4.0)   
4. South Dakota — 69 hospitals, 18 ASCs
5. Louisiana — 240 hospitals, 82 ASCs (2.9)   
6. Alabama — 134 hospitals, 49 ASCs (2.7)   
7. North Dakota — 54 hospitals, 20 ASCs (2.7)   
8. Kentucky — 137 hospitals, 51 ASCs (2.7)
