The 55,000-square-foot office would house eight physicians caring for up to 15 patients at a time. A 20,000-square-foot area dedicated to rehabilitation would be run by Saratoga Hospital and linked to a 35,000-square-foot YMCA complex housing recreation space and a daycare.

The YMCA anticipates 250 people will use the facility daily, according to the report.

The Malta Planning Board is set to review the plan Jan. 17.
