The HIMSS Analytics EMR Adoption Model has eight stages in all, and less than 5 percent of U.S. hospitals have achieved Stage 7 validation.
The certification recognizes improvements the medical center has made to patient care after discharge. The medical center used its EHR to ensure consistent contact with patients in the weeks following discharge. The patient’s primary care team is notified via the EHR when the patient leaves the hospital, and then a care management nurse works with the primary care physician to ensure follow-up.
Previously, the medical center used voicemails and faxes to provide patient care after discharge, according to the report.
