On June 19, the organization became aware that a notebook containing information such as names, dates of birth, patient ID numbers, diagnosis and some treatment information was missing. Affected patients will be notified of their status via first class mail. Cancer Care Northwest is offering assistance in placing fraud alerts on credit reports with their respective bureaus, according to the statement.
The organization stated that no personal financial information was included among the compromised data.
