Health Choice Network taps Wellcentive for value-based solutions: 3 things to know

Health Choice Network has signed Wellcentive to use its population health management products and services across HCN’s 46 health centers.

Below are three things to know about the partnership.

1. Besides population health management and reporting efforts, the partnership will include a collaborative initiative to expand quality measures, value-based insurance design and financial analytics to track and test approaches for treating chronic care patients within underserved communities.

2. HCN’s care network includes federally qualified health centers, physician practices and accountable care organizations serving more than 1 million patients.

3. In a prepared statement, HCN President and CEO Alejandro Romillo said, “Our mission includes providers and locations across 15 states, so it was critical to choose a partner that could both aggregate and normalize data across a diverse care delivery and technology network, and allow us to drive our vision for quality, care management, network integration and optimal patient care.”
