Community Health Partnership deploys Eccovia Solutions’ care coordination platform

Community Health Partnership, a coalition of more than 25 organizations based in Colorado, launched Eccovia Solutions’ community care coordination platform, ClientTrack.

The coalition will use the platform to integrate care for members of its Community Care of Central Colorado program, which coordinates delivery of services for the state’s Medicaid program, Health First Colorado

The platform aims to allow Community Health Partnership to improve access and control cost for the Health First program. ClientTrack can support several government, health and social agencies at the same time, allowing users to gain a comprehensive view of care delivery.

“Using the Fast Healthcare Interoperability Resources data exchange standards, ClientTrack integrates with CHP’s robust data warehouse, ALLURData, and the CORHIO health information exchange,” said Jim Calanni, CHP’s chief technology officer. “This integration enables 25 providers to collaborate and share encounter data in order to refer people for assistance and care from the most appropriate agency for their unique needs, and measure and report on their outcomes.”
