Rural hospital layoffs rise in 2026 as closures fall

Rural hospitals and health systems make up a growing share of the workforce reductions Becker’s has tracked in 2026, even as rural hospital closures have slowed.

Becker’s compared its 2026 lists of hospital and health system job cuts and closures with 2025, covering January through mid-October of each year. Entries were classified as rural if the affected facility, or the system’s headquarters when no specific facility was named, is in a nonmetropolitan county as defined by the federal Office of Management and Budget.

Rural layoffs: more entries, a bigger share

Of the 68 workforce reductions Becker’s has covered in 2026, 10, or about 15%, were at rural hospitals or systems. Over the same period in 2025, fewer than 10% were rural.

The increase came as the total number of job cut entries fell, from more than 80 during that stretch of 2025 to 68 in 2026.

Some 2026 rural cuts affected a significant share of staff. Greenwood (Miss.) Leflore Hospital shared plans in April to lay off 86 employees, about 17% of its workforce, as it prepares for a potential closure or acquisition by a larger health system. Other rural organizations that cut jobs in 2026 include:

  • Montrose (Colo.) Regional Health eliminated 77 positions, effective Oct. 12.
  • Pratt (Kan.) Regional Medical Center was preparing for a “significant workforce reduction” as of July.
  • Magnolia (Ark.) Regional Medical Center laid off about 20 employees, primarily in clinical roles, in March.
  • Ogdensburg, N.Y.-based North Star Health Alliance shared plans to eliminate more than 100 positions in January.

In 2025, the largest cut by share of workforce at a rural organization that Becker’s covered was at Arkansas Valley Regional Medical Center in La Junta, Colo. It laid off about 5% of its workforce in June 2025 as leaders warned about Medicaid cuts in HR 1. Blue Mountain Hospital District in John Day, Ore., cut nine positions that September, also citing potential Medicaid cuts under the law.

Rural closures: fewer than 2025

Rural hospital closures moved the other way. Becker’s tracked four rural hospital closures from January through September 2025, compared with two over the same period in 2026. Sturgis (Mich.) Hospital closed June 19, 2026, after 101 years, saying “a sustainable path forward could not be achieved.”

In May 2026, Bradford (Pa.) Regional Medical Center ended inpatient and emergency services, affecting 238 employees. The hospital said it “is not closing but transitioning to an outpatient health center.”

Distress shifts to service cuts

The slowdown comes as many rural hospitals remain under strain. A January analysis from the Center for Healthcare Quality and Payment Reform found that 734 rural hospitals, about one-third of rural facilities nationwide, are at risk of closure.

The pattern matches what hospital leaders told Becker’s in September. “I don’t think the lower number of rural hospital closures necessarily means hospitals are healthier,” said Brock Slabach, COO of the National Rural Health Association. He said struggling hospitals have turned to alternatives such as rural emergency hospital conversions and service-line reductions, and that distress “shows up as consolidation and service erosion rather than a full hospital shutdown.”

The American Hospital Association also noted slightly more rural emergency hospital conversions so far in 2026 than in 2025, changes that do not appear in closure counts.

Rural hospitals are not alone in facing closure risk. A Harvard T.H. Chan School of Public Health study found that rural and urban hospitals closed at comparable rates from 2010 to 2025.
