Community care organization taps Wellcentive for population management solutions

Portland, Ore.-based FamilyCare Health has selected Wellcentive to provide population health analytics, quality reporting and care management services for the community care organization’s 130,000-plus member base.

Wellcentive’s platforms aggregate patient data, apply analytics to identify patient trends and risks and then deliver action-based workflows to help health systems forecast care outcomes and population care costs.

“FamilyCare is innovating the way payer-based organizations impact health, and this agreement will combine that vision with Wellcentive’s ability and commitment to support the process of value-based care for its providers and members,” said Wellcentive CEO Tom Zajac.

FamilyCare is one of 16 community care organizations working to manage Medicare and Medicaid populations in the state of Oregon. “Our company is excited to work with Wellcentive’s innovative platform for driving workflow and improving health,” said FamilyCare vice president of information services and technology Chris Diaz.
