Evolent Health has rolled out a provider-led operating platform for health systems entering into, managing or growing value-based contracts and provider-sponsored health plans.
Evolent's new platform IdentifiSM 5.0 uses predictive analytics to anticipate the likelihood of specific clinical outcomes. The platform alerts clinicians of these health possibilities at the point of care, aiming to help providers code for risk adjustment and effectively coordinate care.
