The collaboration aims to improve interactions between providers and patients from inpatient to outpatient settings as well as home care. Care teams will be able to connect with patients before and after they’re admitted through automated check-ins as well as identify individuals who need assistance with check-in in near-real time.
Through the partnership, patients will be able to digitally connect with their care members, access their treatment plan and view personalized educational materials to help them prepare for a health event or aid their recovery. These capabilities will be offered to the patient before, during and after a care encounter.
