Change Healthcare launches social determinants of health analytics resource: 3 notes 

Nashville, Tenn.-based data analytics firm Change Healthcare is rolling out a social determinants of health resource to help healthcare organizations identify and examine how social and economic factors affect patient health. 

Three notes: 

1. The Social Determinants of Health Analytics tool is available to health systems, insurers and life sciences organizations across the U.S, according to the Nov. 16 news release. 

2. The resource links deidentified claims with factors including financial stability, education level, ethnicity, housing status and household characteristics to identify correlations between social determinants of health, clinical care and patient outcomes. 

3. With the tool, organizations can receive customized reports identifying social determinants of health factors that affect emergency room, inpatient and outpatient visits across different population health segments and add existing systems with social determinants of health data to close information gaps in care.
