Xerox has launched a full lifecycle population health management solution for healthcare providers.
The population health software, Xerox Health Outcome Solutions, will combine analytics, clinical, technology and administrative services to help providers identify and care for at-risk populations while minimizing financial risk.
Xerox's new solution is currently available to hospitals, accountable care organizations and integrated delivery networks. Xerox hopes next to expand into the payer market.
