Leading the way to achieve the triple aim through clinical integration accreditation

A recent blog post by URAC discusses St. Vincent’s Health Partners becoming the first CIN to achieve Clinical Integration Re-Accreditation

 St. Vincent’s Health Partners (SVHP), based in Bridgeport, Conn., became the first CIN in the country to achieve URAC Clinical Integration accreditation in 2014, and the first CIN to achieve re-accreditation in 2017. SVHP includes more than 400 independent and employed primary and specialty care providers across 85 practice sites, a flagship local hospital and five skilled nursing facilities (SNFs), and four home health agencies and hospices.
