The agreement focuses on preventive care, chronic conditions and disease management, according to the report. It will allow Humana beneficiaries to receive in-network access to inpatient, outpatient and emergency care services at Boulder-based Foothills Hospital, Boulder Community Hospital, the urgent care center and physician offices, in addition to specialty services at Mapleton Center, a Boulder-based outpatient rehab facility, and Lafayette, Colo.-based Community Medical Center. Humana members will have in-network access to more than 128 Boulder Community Health physicians.
