Carilion Clinic (Roanoke, Va.). After forming a patient-centered medical home in 2008, Carilion Clinic was ready to adopt the ACO model in 2011 through an agreement with Aetna. The following year, Carilion Clinic joined Track 1 of the Medicare Shared Savings Program under the name Doctors Connected, and in 2017 it transitioned to the Next Generation ACO Model. Carilion Clinic has also collaborated with Anthem, UnitedHealthcare and Optima Health in pay-for-performance programs.
