Accountable Care Coalition of Chesapeake (Houston). In 2013, Universal American Corp. and a group of primary care physicians collaborated to launch Accountable Care Coalition of Chesapeake. In partnership with Collaborative Health Systems, a Universal American subsidiary, the 200 ACC Chesapeake providers care for more than 28,000 Medicare beneficiaries in Maryland and Virginia. CMS selected the ACO to participate in the Next Generation ACO Model in 2017.
