Joint Commission: 100 Organizations Achieve Primary Care Medical Home Certification

The Joint Commission’s ambulatory healthcare accreditation program has announced that over the last two years more than 100 organizations have earned its primary care medical home certification.

The certification option began in 2011 and uses metrics concerning access to care, patient-centric care, coordinated care and a systemic approach to care quality and safety.

To pursue the primary care medical home certification, organizations must first fulfill The Joint Commission’s ambulatory care accreditation requirements. The certificate is also available for hospitals and critical access hospitals accredited by The Joint Commission and providing primary care services.
