Blue Shield of California, HCA and Santa Clara County IPA sign 3 year accountable care agreement

Blue Shield of California, the Individual Practice Association Medical Group of Santa Clara County and Hospital Corporation of America's Far West Division are launching a three-year accountable care agreement aimed at providing better care to approximately 13,000 Blue Shield HMO members in Santa Clara County.

More than 800 physicians managed by the Individual Practice Association Medical Group of Santa Clara County will provide care to the Blue Shield members. The goal of the agreement is to improve quality of care while cutting healthcare costs, according to a news release.

The organizations participating in the ACO will coordinate care by sharing case management and clinical information.
