10 recently announced accountable care agreements

The following is a roundup of recently announced accountable care organizations or collaborative care agreements, beginning with the most recent.

1. Humana, Iora Health partner for accountable care
Louisville, Ky.-based Humana and Brooklyn, N.Y.-based Iora Health will provide coordinated care for Humana's Medicare Advantage members at four new Iora Primary Care practices — two in Phoenix and two in Seattle.

2. Florida Blue, Baptist Health to launch ACO
Health insurer Florida Blue and Pensacola, Fla.-based Baptist Health Care are forming  ACO aimed at improving quality of care and cutting healthcare costs for patients in northwest Florida.

3. Aetna, Weill Cornell Physicians form ACO in New York
Health insurer Aetna and New York-based Weill Cornell Physicians entered into an accountable care agreement aimed at improving health outcomes for roughly 9,000 of Aetna's commercial and Medicare members in New York.

4. St. Mary Medical Center, 250 physicians form ACO in Pennsylvania
St. Mary Medical Center in LanghornePa., and 250 affiliated physicians formed an ACO that is participating in the Medicare Shared Savings Program.

5. Anthem Blue Cross and Blue Shield, Mount Carmel Health Partners launch ACO in Ohio
Columbus, Ohio-based Mount Carmel Health Partners — a physician-hospital organization — and Anthem Blue Cross and Blue Shield in Ohio are launching a commercial ACO in central Ohio.

6. Delaware Valley ACO to add 50 physicians
Doylestown (Pa.) Hospital and 50 primary care physicians from the hospital's medical staff have agreed to join Delaware Valley ACO.

7. Aetna, Baylor Scott & White Quality Alliance partner for accountable care in Dallas
Health insurer Aetna and Baylor Scott & White Quality Alliance — a comprehensive accountable care network comprised of more than 2,500 physicians — will begin an accountable care collaboration inTexas.

8. The Nebraska Health Network, Humana ink accountable care deal
Health insurer Humana and the Nebraska Health Network are forming an accountable care partnership.

9. Osler Health IPA, Horizon BCBS of New Jersey partner to provide coordinated care
JerseyCare IPA — a subsidiary of Osler Health IPA, a physician owned and managed health network — partnered with Horizon Blue Cross Blue Shield of New Jersey to improve quality of care and lower healthcare costs for Horizon BCBSNJ members beginning Oct. 1.

10. Humana, Health Choice Utah Preferred Network ink accountable care deal
Salt Lake City-based Health Choice Utah Preferred Network and health insurer Humana entered into an accountable care agreement aimed at providing improved quality of care to Humana Medicare Advantage members based in four counties in Utah.
