Osler Health IPA, Horizon BCBS of New Jersey partner to provide coordinated care

JerseyCare IPA — a subsidiary of Osler Health IPA, a physician owned and managed health network — has partnered with Horizon Blue Cross Blue Shield of New Jersey to improve quality of care and lower healthcare costs for Horizon BCBSNJ members beginning Oct. 1.

Osler physicians will use analytics and communication from MDclick — Osler’s proprietary shared population health platform — to coordinate care and help drive down healthcare costs for Horizon BCBSNJ members.

“We look forward to collaborating with Osler Health to help deliver more coordinated care to drive improved health outcomes and a better patient experience for our members,” said Jim Albano, vice president of network management and Horizon healthcare innovations at Horizon BCBSNJ.
