The first step to effective population health management: Improve clinical communications

The buzzword “population health management” is commonly used without clear definition.

In 2003, David Kindig, MD, PhD, and Greg Stoddart, PhD, defined population health as “the health outcomes of a group of individuals, including the distribution of such outcomes within the group.” Increasing emphasis on social determinants of health, disparities in access to healthcare and the triple aim have since influenced this definition.

Population health management, or the efforts healthcare and community organizations take to improve the health of a population, is comprised of both clinical and nonclinical elements. For clinicians, an essential element is seamless communication between clinicians in different care settings to improve patient outcomes.
