Under the home model, physicians will be incentivized for improving health, affordability and patient experience. RCCA will be responsible for managing care and a personal physician will oversee each patient and coordinate his or her care with other professionals.

The oncology medical home is a shift from a fee-for-service payment model to a value-based payment model.
