Cybersecurity threats, climate change and financial pressures are threatening  hospital supply chains, according to the American Society of Health-System Pharmacists’ “2026 Pharmacy Forecast,” released Dec. 9.

The report, presented at ASHP’s Midyear Clinical Meeting in Las Vegas, draws on insights from nearly 300 pharmacy leaders, according to a Dec. 9 news release from the organization. Two-thirds said a cyberattack could disrupt drug delivery nationwide, and nearly three-quarters said they anticipated weather-related events would interfere with hospital operations and medication access.

The forecast outlines strategies to strengthen supply chain and financial resilience, including investing in domestic production, shifting away from cost-only decision-making and building transparency into supply chain partnerships. It also calls on health systems to treat sustainability and infrastructure planning as “strategic imperatives, not optional projects.”

“Pharmacy leaders are uniquely positioned to bring solutions to their organizations as we prepare for disruptions that will define the healthcare system of the future,” ASHP CEO Paul Abramowitz, PharmD, said in the release.

The report also called attention to financial pressures stemming from 340B and prescription benefit management policy changes, uncompensated care, and underscored the need to prepare for new therapies and digital transformation.
