The New Mexico Health Care Authority announced a $76.2 million federal funding opportunity through Healthy Horizons, a component of the state’s Rural Health Transformation Program, to expand specialty, maternal, behavioral health and chronic disease care.
The Santa Fe-based authority said June 3 it will select six organizations to manage hub regions and help communities plan and execute local healthcare projects. The hubs will convene providers, Tribal health programs, community organizations and public health groups to identify needs, build regional plans and direct funding to partners closing gaps in care, rather than delivering all services directly.
“Rural health needs are different across New Mexico, so the solutions need to be different too,” said Elisa Wrede, acting rural health director. “Through Healthy Horizons, we are investing in regional partners who can bring providers, Tribal health programs, community organizations, public health leaders, and others together to improve access to care in practical ways.”
Each hub must direct at least 90% of its award to local projects, which may target workforce development, telehealth, health IT, care coordination, and referral and transportation barriers. Applications are due July 2 through the program portal.
The funding flows from New Mexico’s $211.5 million Rural Health Transformation Program award, financed entirely by CMS and authorized under HR 1. It builds on the state’s earlier rural push: in January 2025, New Mexico distributed $40.6 million to 26 organizations through its state-funded Rural Health Care Delivery Fund. The five-year federal initiative targets long-term, community-driven solutions across the state’s rural health systems.
